Research Information Sheet — Industry Doctor Survey

You are invited to participate in a MBBC research study: "Workplace Illness Trends and Productivity Impact in the Bangladesh RMG Sector — Clinical Perspective."

Purpose: To gather clinical observations from factory-based doctors and medical officers regarding occupational illness patterns, diagnosis trends, patient load, and the health impact on worker productivity.

Who should complete this: Doctors, medical officers, nurses, and paramedics who provide healthcare services in RMG factory clinics or are assigned to industrial health facilities.

Confidentiality: All responses are completely confidential. No names, factory identities, or personal data will be published. Data is used for academic research only.

Voluntariness: Participation is entirely voluntary with no consequence to your professional standing.

Duration: Approximately 15–20 minutes.

কে পূরণ করবেন: চিকিৎসক, মেডিকেল অফিসার, নার্স এবং প্যারামেডিক যারা আরএমজি কারখানার ক্লিনিকে বা শিল্প স্বাস্থ্য সুবিধায় স্বাস্থ্যসেবা প্রদান করেন।
